Respite Care
Respite Care

It can be rewarding to care for a loved one at home. We also know it can be overwhelming. It is important to your health to take an occasional respite from the responsibility. With Carol Health, you can have a nurse visit your home to care for your loved ones for up to four hours – which can help reduce your stress, restore your energy, and keep your life in balance.

Wound Dressing Change
Wound Dressing Change

Carol Health offers bandage replacement and wound inspection services for surgical incisions, large burns, or even just bad cuts or scrapes. Our trained nurses can also evaluate dressings for infection, check feeding tubes, change colostomy bags, or check catheter sites. Please make sure you have the necessary supplies prior to the visit.

Vital Sign Check
Vital Sign Check

To save you a trip to the doctor’s office, a Carol Health nurse can come to your home to check vital signs – including blood pressure, temperature, respiratory rate, pulse, and blood oxygen saturation. Tracking vital signs can determine treatment procedures, provide feedback on the effectiveness of treatments performed, and equip medical professionals with critical information to make lifesaving decisions.

Medication Assistance
Medication Assistance

Carol Health can make sure your prescribed medications are set up properly and according to your doctor’s orders. To help you understand them even better, we can explain the purpose and proper use for each medication and even work with your insurance company to make sure they’re correct for your needs.

Medication Reconciliation
Medication Reconciliation

Since errors with medications are frequent and potentially dangerous, medications should be "reconciled" – that is, the pre-hospitalization medications should be compared with the post-discharge list to see that there are no duplications, omissions, or harmful side effects. Carol Health can help you manage medications upon discharge, as well as changes to medications that may follow.

Safe Home Assessment
Safe Home Assessment

The home environment can greatly impact a person’s recovery. Carol Health nurses will help you determine the best home modifications to ensure your home is accessible and safe – finding and removing obstacles and checking for potential dangers like tripping hazards, which will minimize the risk of fall, injury, hospitalization, and long-term placement.

Discharge Instruction Review
Discharge Instruction Review

A successful transition home – whether the discharge is from a hospital, a rehabilitation facility, or a nursing home – is critically important to a healthy recovery. Good discharge planning can decrease the chances of readmission, help aid in recovery, and ensure medications are prescribed and given correctly. Our nurses will work with you and your family to adequately prepare for the best possible care at home.

Basic Health Assessment
Basic Health Assessment

A Carol Health nurse can perform a general evaluation of a patient’s overall health – not only for reassurance, but for guidance on continuing care. We will evaluate charts and vital signs and test reflexes, breathing rates, blood pressure, heartbeat, and other important health indicators to identify current and future health needs.
